چکیده

Abstract Background IL-33/ST2 axis and gut microbiota are important factors in the pathogenesis of IBD with a potential reciprocal influence. controls mucosal healing process during intestinal inflammation. Pouchitis, after restorative proctocolectomy ileal pouch-anal anastomosis, is most common complication. We aimed to explore correlation between cytokines inflammatory pouchitis. Methods 23 patients anastomosis were enrolled. 13 showed pouchitis 10 normal pouch. Mucosal samples collected from afferent loop Multiplex analysis for was performed. ELISA, western blot IHC run assess axis. Microbiota analyzed by 16S ribosomal RNA gene amplicon pyrosequencing. Pouchitis Disease Activity Index Histological Score calculated. Multivariate analyses generate transcriptional interaction networks identify biomarkers inflamed pouches. Results IL-1Ra, IL-6, IL-8, IL-17, IP-10, MCP-1, MIP-1a, MIP-1b increased pouch vs. No differences registered loop. In pouchitis, IL-7 resulted reduced pouch, no recorded. IL-33 ST2 protein levels significantly decreased Full-length, bioactive IL33, ST2L sST2 expressed all experimental groups; cleaved, less active form IL33 only stainings intense within ulcerated mucosa compared close proximity areas re-epithelialization. biodiversity (both at level). Lower Verrucomicrobia abundance Actinobacteria recorded vs Akkermansia muciniphila augmented Collinsella multivariate evidenced significant inverse e aereofaciens levels. Conclusion characterized divergent molecular pathways microbial populations specific pattern Our results suggest role driving wound anastomosis.

Techniques of laparoscopic total proctocolectomy and ileal pouch anal anastomosis patients with ulcerative colitis

The incidence of ulcerative colitis (UC) is increasing in adults and children worldwide. Despite the advances in drug therapy, surgery is also an important treatment. Total proctocolectomy (TPC) with ileal pouch anal anastomosis (IPAA) is the gold standard surgical treatment of UC since its introduction in 1978 by Parks and Nicholls. Laparoscopic TPC with IPAA, as a new approach for UC patients...

Ileal-pouch-anal anastomosis after restorative proctocolectomy in patients with ulcerative colitis or familial adenomatous polyposis.

BACKGROUND/AIMS Restorative proctocolectomy is the "golden standard" in surgical treatment of ulcerative colitis and familial adenomatous polyposis. The two alternative techniques of ileal-pouch-anal anastomosis include hand-made suture and double line stapled suture. The aim of the study was the analysis of postoperative complications and functional results of the two types of anastomosis. M...

Characterization of the Gut-Associated Microbiome in Inflammatory Pouch Complications Following Ileal Pouch-Anal Anastomosis

INTRODUCTION Inflammatory complications following ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC) are common and thought to arise through mechanisms similar to de novo onset inflammatory bowel disease. The aim of this study was to determine whether specific organisms in the tissue-associated microbiota are associated with inflammatory pouch complications. METHODS Patients havi...

Outcome of total proctocolectomy with ileal pouch-anal anastomosis for ulcerative colitis

PURPOSE We evaluated the risk factors for late complications and functional outcome after total proctocolectomy (TPC) with ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC). METHODS Pre- and postoperative clinical status and follow-up data were obtained for 55 patients who underwent TPC with IPAA between 1999 and 2010. The median follow-up duration was 4.17 years. Late complicat...

Genetic and Demographic Correlates of Quality of Life after Ileal Pouch Anal Anastomosis for Ulcerative Colitis

Objective: Patient satisfaction after ileal pouch anal anastomosis (IPAA) for ulcerative colitis (UC) is difficult to predict preoperatively and has never been investigated from a genetic perspective. Methods: Modified IBD quality of life (QOL) questionnaires were mailed to all UC-IPAA patients in our IBD Biobank. Genotyping was performed using a custom microarray containing 325 IBD-associated ...
